Financial Assistance

If you need help paying for your medications, healthcare services, or dental services, you may qualify for these programs!

Medication Assistance Program

This program was started to help patients that are unable to afford their medications.  If you currently have Medicaid, Medicare Part D, or any insurance that pays on your medications, you will not be eligible for this program.  All applications must include proof of income.  The application is provided for you below.  If you have any questions, please call (828) 754-2409 Ext. 102.

Sliding Fee Scale

To help ensure access to quality healthcare for all patients, West Caldwell Health Council, Inc. offers a sliding fee scale to patients according to their income and household size.  Sliding fee scale applications are offered at any of our locations.  You may apply regardless of whether you have other insurance coverage or not. Patients must complete/sign the application and provide proof of their total household income as required by federal regulations.  If you would like to apply, please fill out the following application and provide the documentation required.  If you have any questions, call (828) 754-2409 Ext. 104.

Dental Program
